Patellar Tendonitis is an overuse injury of the knee and makes up 20% of all knee injuries. Additionally, 1 out of 10 adolescents suffer from Osgood Schlatter’s Syndrome. The location of pain with Patellar Tendonitis is on the front of the knee just below the kneecap (patella) along the tendon, down to the bump on the front of the upper shin (tibial tubercle).

This irritation may be brought on by several factors such as trauma, muscle imbalance, weakness in the hips or ankles, repetitive activities such as running, squatting, climbing, post-surgical sequela, or with Osgood Schlatter’s Syndrome, a common condition experienced during a rapid period of growth.

Diagnosis which present with pain located in the front of the knee and patellar tendon include Patellofemoral Syndrome, Chondromalacia Patellae, Inferior Patellar Fat Pad Syndrome.

Symptoms of these disorders are described as pain, localized about front of the knee brought on by passive flexion (bending), or resisted extension (straightening). Pain is commonly worse with squatting, stairs (worse on descent) and standing from a seated position.

Treatment may include non-steroidal medication, rest, activity modification, ice, stretching, and gradual controlled strengthening exercises as directed by a physical therapist or athletic trainer. These typically include working on the muscles of the hip, ankle, while slowly introducing the quadriceps.

In addition to these treatment options, occasionally a strap or tape is used in order to provide support and assist in dispersing the forces about the knee. Unfortunately, these devices utilize a flawed rationale in that they apply pressure to the irritated tissue of the tendon. Research has shown that compression is not healthy for any tendon and typically causes further tissue damage.
